Built on public federal payer-rate data, no PHI, ever. See how it works
PracticeOS · Reimbursement Autopilot

Your contracts. Audited, renegotiated, and monitored — automatically.

PracticeOS is the reimbursement department your independent practice never had. We read every payer's federally-mandated rate file, audit every contract, draft every leverage memo, and watch every renewal. You enter your NPI. We do the rest.

We do the work. You keep the money.

PracticeOS is not a tool you have to learn. It is not a dashboard you have to babysit. It is a fully autonomous reimbursement layer that runs in the background of your practice. Every month, every payer, every CPT code, every renewal — we are awake on it. You get a summary in your inbox. You sign the leverage memos we drafted. You watch what comes back in.

01
You give us your NPI
Ten digits. Public information. We pull your practice profile from NPPES and identify every payer you're contracted with from federal Transparency-in-Coverage filings.
02
We audit every contract
Every CPT code, every payer, against the peer benchmark in your locality. You get a Practice Audit and a documented opportunity figure in writing — not someone else's average.
03
We draft the leverage memos
For every underperforming contract, PracticeOS generates a formal rate-realignment request citing the payer's own federal filing. You review, sign, and submit. We track the response.
04
We watch it forever
Federal rate files refresh on the 1st of every month. So do we. New gaps surface, new renewals queue, payer behavior changes — we alert you. You don't have to remember any of it.
The opportunity · 2026

A federal mandate created the largest hidden rate dataset in healthcare. We turned it into a product.

In July 2022 the federal Transparency-in-Coverage rule went live. Every commercial payer was required to publish every negotiated rate, by provider, by code, every month. The files are usable only by entities with serious infrastructure. ReimburseOS built that infrastructure for the independent practice.

Market Trigger
45 CFR
Part 180

The federal rule that forced every commercial payer to publish every negotiated rate. Effective July 1, 2022. Refreshed monthly.

Data Universe
314M+
rate rows

Raw federal payer rate records across 254 commercial payers and 50 states — normalized into a queryable rate book.

TAM (US)
~9.2M
NPIs

Active NPIs in the NPI Registry. Hospital-scale rate intelligence is built. Independent-practice-scale was not, until now.

The Wedge
SMB
healthcare

Rate intelligence built for the independent practice, the billing firm, and the DMEPOS supplier — not the hospital system.

Why now

The transparency rule made the data legal to use. The cost of infrastructure made it unusable for everyone except an engineering team. PracticeOS closed that gap, and the federal data refreshes monthly — forever.

What the gap actually looks like.

Every figure below is documented from public Transparency-in-Coverage rate files against locality peer benchmarks. Actual recovery depends on payer response, contract terms, documentation, and negotiation outcome.

$172,200 /yr
Solo Physical Therapist · CA

17 CPTs benchmarked. 4 payers below locality mid. Largest gap: 97140 manual therapy at 71% of locality median. Practice Audit delivered. Leverage memo drafted for top three payers.

$284,000 /yr
Orthopedics Group · 4 NPIs · TX

Cigna renewal in 28 days when Autopilot flagged. Brief drafted automatically using the payer's own federal filing. Three CPTs (99213, 20610, 27447) were below locality mid by a combined $284k annual.

$2.1M /yr
Billing Co · 47 NPIs · multi-state

Aggregate documented opportunity across the client roster. Sorted by NPI, by payer, and by renewal window. White-label deliverables sent to every client. Average client uplift: ~$44,500/yr.

We stake our fee on the gap. Either we find your specialty floor in documented opportunity — or every dollar comes back.

The Founder's Promise floor scales by specialty and practice size. We will not invoice for a Practice Audit that doesn't clear that floor in documented opportunity from public payer data. That's the deal — written, signed, and refundable.
